New MDS RAI Manual

CMS has posted a New MDS RAI Manual. MDS 3.0 RAI Manual Update, May 25, 2011 (zip file contains 58 files). Included in this zip file is the complete manual as well as contains all manual chapters/sections/appendices/title page/Table of Contents/forms. Click here for a high level overview of the changes.
Files containing revisions in this update include: Title Page, Table of Contents, Chapter 1, Chapter 2, Chapter 3 (Introduction, Sections: A, B, D, F, G, I, J, K, M, N, O, and Q), Chapter 5, Chapter 6 and Appendices (A, B, C, E, and H). Files from earlier versions (e.g., September 2010) are contained in the manual but do not have an associated change table file.
When reviewing the new manual, note that the month/year located at the bottom left on the manual pages reflects when that file was last updated (e.g., May 2011 for this latest version).

A Modification Request should be used when an MDS record (assessment, Entry tracking record or Death in Facility tracking record) is in the QIES ASAP system, but the information in the record contains clinical or demographic errors.
The Modification Request is used to modify most MDS items. The exceptions are:
- An Inactivation of the existing record followed by submission of a new corrected record is required to correct:
- Type of Provider (Item A0200)
- Type of Assessment (A0310)
- Entry Date (Item A1600) on an Entry tracking record (A0310F = 1),
- Discharge Date (Item A2000) on a Discharge/Death in Facility record (A0310F = 10, 11, 12),
- Assessment Reference Date (Item A2300) on an OBRA or PPS assessment.

In addition, CMS has also updated the complete list of all RAI MDS Coordinators, by state.

HMX will be updating Vision MDS in accordance with these changes and will release as a script when development is completed.
